Just reviewed the final records on a patient being seen by a doctor at the Umpqua Community Health Center. I had previously reviewed what records they sent and had to ask for additional records that they failed to send as part of the original package.
It turns out that they don’t have what I requested!
NO imaging on file for the low back pain they are prescribing for!
NO recommended screening questionnaires BEFORE starting chronic opioid treatment!
NO pain agreement was ever done with the patient!
NO Material Risk Notice was ever done too!
These are ALL recommended and advised to be done by multiple medical associations and the Oregon Medical Board.

Per a letter I wrote awhile back, I see that the Oregon PDMP is finally reporting the refills properly per my suggestion.
If they want to claim religious beliefs, then they need to be willing to pay the consequences of such beliefs too.
Pulled out some records to check some things and refreshed myself with something I had been meaning to post earlier but got distracted.
I find it amazing that so many clinics, especially pain clinics are ignoring the requirements of having pain agreements and Material Risk Notices with their patients.
